About CBHI Pearaing

CBHI is the abreviation of Community Base Health Insurance. CBHI Pearaing is a new project implemented by Reproductive And Child Health Alliance (RACHA) with Budget and Technical support from Health Net TPO since the end of 2009 until now. The name of the scheme is My Family’s Health, សុខភាពគ្រួសារខ្ញុំ, is a popular name for community and it is easy to call and remember.

History of Scheme

• In 2004, HealthNet International initiated to implement CBHI Scheme in Pearaing Operation District, Prey Veng Province.
• In 2008, HNI made a survey in the district to know about the market price and population’s interest.
• In 2009, HNI made MoU with RACHA as partner of implementation.
• In February 2010, key staffs are recruited and trained in India.
• In June 2010,  RACHA had contracted with 3 health centers _ Poty, Prey Pnov and Kampong Popil, Pearaing referral hospital and Russian Hospital (Phnom Penh).
• In January 2011, the scheme expanded to other 6 health centers in this OD.

The Main Objective

• To reduce poverty of community by reducing pocket expenditure of health care.
• Help people to save their money and teach them how to prevent their health from illness and how to be hygiene.
• To increase quality of Public Health Service
• To reduce maternal and under 5 year mortality
• To increase modern contraceptive prevalence

Benefits My Family’s Health provide:

- Health Care: 3 level of health care are provided to members with free of charge. They can get consulting and treatment and all service existing in Health Center near their house in 24 hours. In case of serious disease, patients are referred to Pearaing Referral Hospital and if the RH cannot treat the patients are referred to Khmer-Soviet Friendship Hospital in Phnom Penh. All the expense of treatment are paid by MFH Scheme.

- Transportation: In case of emergency, MFH pay for Ambulance from Health Center to Referral Hospital and from Referral Hospital to Khmer-Soviet Friendship Hospital. If it is not emergency case, MFH pay for members according to travel allowance policy of MFH.

- Food Allowance: During admitting in Hospital, MFH support food expense for patients and care takers for unlimited days per year.

- Grant: In case member died in the hospital, MFH pay for refer body back home and give 50,000r as grant in funeral. (USD1=4,000r).

*Benefits for Pregnant Women

To reach the goal of reducing maternal and new born mortality, MFH encourage pregnant women to become member by providing some benefits below:

1. Discount 10,000riels when the first enrollment at any time during pregnant and she can delivery for free at health center or referral hospital.
2. Give reward 10,000riels in 9 months later if she completes some conditions below like: before delivery she got Antenatal Care at least 4 times, took FER at least 90 tablets, and delivery at Health Center. After delivery she get Post Natal Care at least 3 times, take FER at least 42 tablets, breast feeding in 6 months without additional food or drink, and complete vaccination.
3. Give reward 5,000riels more if she use any service of Birth Spacing.

The Condition to Become member

1. Enroll only people in catchment area
2. Member enrolled as family not household according to Family Book
3. Premium payment:

• Family with 1 member        : 36,000 riels​ per year
• Family with 2-3 members : 62,400 riels per year
• Family with 4-6 members : 76,800 riels per year
• Family with 7 members up: 88,800riels per year

The Challenge

1. Patients are always ask for refer to Referral Hospital. They rather do not trust on Health Center treatment or they want to earn money from travel and food allowance.
2. There is no yet a facilitator at Russian Hospital which is rather difficult for patients who go there to find and communicate.
3. Two different implementers of CBHI and HEF, which can not link together. Sometimes, poor want to become member of CBHI, but we can not enroll.

Lessons Learned

1. People can now know Referral Hospital after they become CBHI member. Before they mostly go to private sector when they have health problem. Mean that the service of public health sector is increased.
2. Member of CBHI are decreased expenditure from health care.
3. Full village meeting is a potential way of promotion, which spend less and get good result.
4. Completely cover of travel expense rather encourage patient ask for refer even though their sickness is not necessary.
5. Use Fee for Service to pay for hospital rather force physician like to use Para-Clinic examination unnecessarily.

The Future Plan

1. In the 4^th quarter 2011, RACHA-MFH is going to expand to other six health centers in Pearaing OD (coverage the whole OD = 15 HCs)
2. In 2012, RACHA-MFH will survey other three ODs in Prey Veng Province _ Preah Sdach, Kampong Trabek and Mesang.
